INCIDENT REPORT

 

One of the book shelves fell over on February 17, 2022, at approximately 8:30 am at the school library within the premises of Assumption College of Nabunturan. The incident involved the following individuals: Josh Cinco, a second-year Education student; Rose Lopez, a first-year BPA student; and Josephine Plaridel, a third-year Education student who had a minor injury. The library staff member, Ms. Francisca Gomez, witnessed the incident and immediately called the medical team, headed by Mr. Rafael Sanchez, the response team president. 

To summarize, it is critical to develop safety measures and transmit reminders to students in order to avoid these events. The school should evaluate bookshelves and other library furniture on a regular basis for signs of wear and tear or potential risks. Check that the bookshelves are stable, properly affixed to the walls or the floor, and can support the weight of the volumes.
